Temporary health concerns such as injuries, prolonged illness or mental health crises occasionally require short-term accommodations. Students who wish to have their health concerns considered for short-term accommodations should complete the Temporary Accommodations form. Upon submission, the student will be asked to provide current documentation of the temporary health concern, which needs to include the diagnosis, date of evaluation, probable date of release (if known), should appear on the provider’s letterhead and must have a valid signature.
