Academic accommodations support students with both visible and non-visible disabilities by removing barriers in the classroom and/or providing additional routes for students to fully access the curriculum.
In order to begin the process for academic accommodations, please complete two steps:
- Complete the Student Information form
- Submit your documentation (via email, in person, mail, or fax)
If you are not sure if your current documentation meets the requirements, we recommend that you first communicate with us about your current materials before making plans to get new documentation.
If you already have arrangements to meet with a licensed professional, you are welcome, but not required, to use this disability verification form to ensure the essential information is provided from your practitioner.
Documentation Requirements
- Must be typed or printed on official letterhead and signed by an evaluator qualified to make the diagnosis (licensed physician, psychologist, psychiatrist or therapist)
- Must clearly state diagnosis with DSM code or chronic health condition
- Must be current – i.e. completed within the last year for psychiatric disabilities, physical disabilities and chronic health conditions; or within the last five years for learning disabilities, ADHD, Autism Spectrum Disorder and intellectual disabilities. Documentation of traumatic brain injury must have been completed within the past one to three years, depending on the date of the injury.
