  • Request by mail (to office listed above)
    Include check/money order payable to Kentucky State Treasurer
    Allow 30 working days

    Allow 3-10 business days
You will need the following items:
  • Your full name
  • Date of birth
  • County of birth
  • Social Security Number
  • Mother’s full name, including maiden name
  • Father’s full name
  • Mailing address
  • Telephone number
