Print and submit the Application for Disability Services. Accommodations must be determined by your healthcare professional based on your individual disability needs and written in a documentation letter. This letter should be on letterhead and include the health professional’s credentials. Ask your healthcare professional to provide information such as your disability diagnosis, date of the diagnosis, how your disability affects your major life activities and what accommodations support your performance in/outside of the classroom. As a supplement to your healthcare professional’s letter, you may submit your individualized education program (IEP) or Section 504 plan, if applicable, as it may identify services that have been effective for you. Please submit by mail, fax or scan to Disability Services.
If a student has reason to believe their access has been denied they are encouraged to utilize the mechanisms in place to bring about a timely resolution. Grievance Process
